To revive a withered orchid: three days after the flowers fall off, try trimming it down to the second node at the diagonal. If it doesn't grow, cut it back down to "square one" and the flower should start growing again. However, you should never, ever cut the aerial roots off: these are the slightly grey, curling roots around the top.
